If you meet these requirements please call and let the organization know you were screened by ACS which may reduce your eligibility wait time. Registration is limited to the first 25 new patients per weekAdditional comments: Reduced cost services if meet financial guidelines. a. There is a 2.00 charge for registration and a $2.00 administrative fee for each visit thereafter. b. There is a $5.00 registration fee required for dentures. c. There is a $5.00 registration fee for orthodontic services for children.
